I'd go with Clemson myself. It does have a better academic reputation than East Carolina. However, if you have received funding at one over the other, this might be a factor. Do you have 2 years of guaranteed funding at one school and only 1 at the other? Do you have to maintain a 3.5 or a 3.25 GPA? Your student VISA will be tied to ability to pay tuition without working (outside the university), so this might be a factor in your decision. As for future prospects... if you can find a job with Visa sponsorship upon graduation, then you can stay as long as you would like (and, if you are fluent in Spanish, there is a need for bilingual health care workers, so sponsorship will be relatively easy). If not, then you will have to return to your home country. However, if you are looking for a Ph.D. after your Masters, then either school will help you to this end. Job prospects are much better with a Masters degree over a Bachelors in this field.
